As part of the City of Pinellas Park’s continued efforts to improve customer service, we have added a new online portal for common utility billing and customer service needs.

Using the new online portal, you can request new residential or commercial utility service, submit a pool fill or leak affidavit for service credit, and enroll in automatic bill payment.

IMPORTANT: You will need adobe reader to fill out and sign the customer service forms. A free download of adobe reader is available. Please select the appropriate version for your device:

Residential Utility Service Application – Please fill out this form if you would like to establish residential utility service.
Commercial Utility Service Application – Please fill out this form if you would like to establish commercial utility services.
Pool Fill Affidavit – Please fill out this affidavit prior to requesting a pool fill service credit.
Leak Affidavit – Please fill out this affidavit to request a sewer leak credit.
Fire Hydrant Service Application – Please fill out this form to have a fire hydrant/construction meter installed
Automatic Bill Payment Application – Please fill out this form to enroll in automatic utility bill payments.
Reclaimed Water Service Application - Please fill out this form if you would like to apply for reclaimed water service

Once you download the form that you need, please fill it out and submit it to the City of Pinellas Park by creating or logging into your customer service account and then click Utility Billing, Service Request.
